Alpha Omega Life Care, Inc. presently operates and offers the following programs:
CHOICE Waiver program provides the client with in home services. The client must be 65+ or a disabled
adult from 19 - 64 years old. These services allow a client to remain at home in lieu of admission to a nursing home facility.
Waiver services include:
* Care Coordination - develops plans of care, monitors change and provides scheduling and oversight
of service provision for the Medicaid CHOICE waiver program for elderly and disabled adults. Interfaces with other agencies.
* Chore Services - assisting clients who require and qualify for supportive services in order to
remain in their own homes, rather than in nursing facilities.
* Respite Services - providing a short term "break" for a primary caregiver, who might otherwise
become saturated or exhausted and lose efficiency as a caregiver.
* Transportation Services - local transport to market, medical, cultural, religious and social /
community activities.
* Medicaid Non-emergent Transportation - long distance transport to medical appointments with escort
services provided as required.
* Telephone / Email Companion - a voluntary telephone and internet support companionship program.
* Fee Agent Services for the Division of Public Assistance (DPA) - acting as a local agent for
the state DPA office in rural or remote areas.
* Consumer Directed Personal Care Services (CDPCA) - arranging for personal care assistant
services for qualified recipients. The CDPCA program allows the consumer or their personal representative to choose the PCA
that they would like to have work for them and allows the consumer to manage their own personal care. Alpha Omega handles
the administrative elements of the program and provides oversight and payment to the PCA for the Activities of Daily
Living provided to the consumer.
* Hospice - end of life support by trained volunteers.
* Personal Care Assistant Training - certified trainer on staff.
* Job Coaching / Job Development - contracted with the Division of Vocational Rehabilitation.
* Fingerprinting / Passport Photos - user fees to support programs.
* Home Health Equipment Lending Locker - provide equipment on short-term basis e.g. walkers, wheelchairs,
hospital beds etc.
* Commodities Supplemental Food Program (CSFP) - monthly food boxes for eligible seniors ( 60+ years
old), pregnant and postpartum women not currently on WIC, children 1 - 6 years old not currently on WIC and qualifies within
the annual income guidelines.
* Temporary Emergency Food Assistance Program - must qualify within the annual income guidelines;
each household will only be served once a month; the quantity of food is limited to the verified household size. Income
Guidelines for 2006-2007:
For a single individual - monthly earnings $1,889 or annually $22,663;
For two - monthly $2,544 or annually $30,525; for a family of three -
monthly $3,199 or annually $38,388; a family of four - monthly $3,855 or annually $46,250 and for
five - monthly $4,510 or annually $54,113.
